Treosulfan-based conditioning vs. low-dose busulfan-based conditioning for allogeneic hematopoietic stem cell transplantation: a cost-utility analysis in Poland

چکیده
Introduction: In Poland, busulfan conditioning is used for allogeneic hematopoietic stem cell transplantation (allo-HSCT). Cost-utility analyses comparing alternative regimens in patients undergoing allo-HSCT have not been conducted so far. Material and methods: A United Kingdom-based partitioned survival model was adapted to the Polish setting compare treosulfan low-dose regimen from public payer’s perspective Poland. Patient characteristics, overall (OS), event-free (EFS), rate of adverse events were obtained randomized MC-FludT.14/L trial. Parametric models up 5 years (the cure threshold), with subsequent mortality defined using general population Poland adjusted cancer survivors, extrapolate OS EFS beyond trial duration. Published utilities age age-dependent utilities. The costs treatment, events, inpatient/outpatient care assessed via official remuneration schemes. Results: Treosulfan-based outperformed busulfan, i.e. it more effective incremental quality-adjusted life (QALY) 0.78 less expensive by 1,139 PLN per patient over lifetime horizon. Deterministic sensitivity revealed highly cost-effective (i.e. cost-utility ratio lower than gross domestic product capita Poland) compared if most uncertain parameters are changed or scenarios implemented. probability being a threshold 155,514 99.6%. Conclusions: Compared ineligible standard regimens.

Treosulfan and fludarabine: a new toxicity-reduced conditioning regimen for allogeneic hematopoietic stem cell transplantation.
New conditioning regimens are being explored to reduce toxicity and enable allogeneic bone marrow transplantation in patients not eligible for conventional transplantation. We have investigated treosulfan, an alkylating agent, with the aim of developing an efficient and reliable but less-toxic conditioning regimen. A series of 30 patients who were not eligible for standard conditioning therapy ...
متن کاملPersonalized busulfan and treosulfan conditioning for pediatric stem cell transplantation: the role of pharmacogenetics and pharmacokinetics.
Busulfan- and treosulfan-based conditionings are the cornerstone of pediatric allogeneic hematopoietic stem cell transplantation (HSCT). Although both drugs are alkylating agents, their mechanisms of action, pharmacokinetics (PK) and toxicity profiles are different. Experience with busulfan in pediatric HSCT is broad and the knowledge on the pharmacodynamics (PD), PK and, to a lesser extent, ph...
متن کاملAllogeneic hematopoietic stem cell transplantation (HSCT) after reduced intensity conditioning.
Allogeneic hematopoietic stem cell transplantation (HSCT) following myeloablative (conventional) conditioning regimen is associated with a high incidence of transplant-related morbidity and mortality, limiting its use to younger patients without medical co-morbidities. Over the past few years, it has become more evident that the alloreactivity of transplanted donor immunocompetent cells against...
متن کاملTreosulfan-Based Conditioning Regimen in Sibling and Alternative Donor Hematopoietic Stem Cell Transplantation for Children with Sickle Cell Disease
BACKGROUND Sickle cell disease is a genetic disorder involving a defect in the red blood cells due to its sickled hemoglobin. The main therapeutic interventions include preventive and supportive measures. Hematopoietic stem cell transplantations are carried out with the aim of replacing the defective cells and their progenitors (hematopoietic (i.e. blood forming) stem cells) in order to correct...

PURPOSE A fludarabine-based "nonmyeloablative" preparative regimen was investigated in 42 patients with hematological malignancies receiving hematopoietic stem cell grafts from unrelated volunteer donors. EXPERIMENTAL DESIGN Recipient conditioning consisted of fludarabine 30 mg/m(2) on days -6 to -2 and i.v. busulfan 3.3 mg/kg on days -6 to -5. Antithymocyte globuline was added at 2.5 mg/kg i...
